Step 1 Qualification
Applicants must hold a Pre-Contract Examination for Insurance Agents (PCEIA) certificate from Malaysian Insurance Institute or other relevant qualifications. If you wish to know more about PCEIA or would like to take this examination, click here to find out more.
Step 2 Agency Application & PIAM Application for GIARR
Contact our nearest branch to obtain the "Application for Agency" and "PIAM Application Form For Registration of General Insurance Agent". If you want our representative to contact you, click here to submit your contact details.

Complete and submit the forms together with the relevant documents according to type of applicant as shown below.

Individual   Individual
For individual applications, you will need the following documents:

Agency Application
  1. Minimum 2 Third Party Guarantors
  2. Agency Application Form
  3. Photo copy of Identity Card
  4. Screening Requirement - Consent Letter (duly signed by applicant)
  5. 2 sets of completed Agency Agreement

All guarantors are required to submit the following documentation for agency application.

  1. Photo copy of Identity Card
  2. Salary Slip / Income Tax J Form / Bank Statement
  3. Particular of Guarantor(s) form duly completed
  4. 2 sets of completed Guarantee & Indemnity


PIAM Application for GIARR
  1. PIAM Application Form For Registration of General Insurance Agent (first time registration) or PIAM Identification Card & Release Letter from previous principal if applicable (2nd inclusion)
  2. PCEIA (MII Certificate / Result Slip) / Any qualification stated in Application of the GIARR
  3. Postal Order / Money Order / Banker's Cheque (RM15.00)
  4. SPM certificate (min) or equivalent / Statutory Declaration on Lost of SPM certificate (first time registration)



Sole Proprietor   Sole Proprietor
Sole proprietor application requires the following documentation:

Agency Application
  1. Minimum 2 Third Party Guarantors
  2. Agency Application Form
  3. Photo copy of Identity Card
  4. SSM Registration / Renewal form (Borang D)
  5. Borang A / B or Suruhanjaya Syarikat Malaysia (SSM) form
  6. Screening Requirement - Consent Letter (duly signed by corporate nominee / proprietor)
  7. 2 sets of completed Agency Agreement

All guarantors are required to submit the following documentation for agency application.

  1. Photo copy of Identity Card
  2. Salary Slip / Income Tax J Form / Bank Statement
  3. Particular of Guarantor(s) form duly completed
  4. 2 sets of completed Guarantee & Indemnity


PIAM Application for GIARR
  1. PIAM Application Form For Registration of General Insurance Agent (first time registration) or PIAM Identification Card & Release Letter from previous principal if applicable (2nd inclusion)

  4. SSM Registration / Renewal form (Borang D)
  5. Borang A / B or Suruhanjaya Syarikat Malaysia (SSM) form
  6. Screening Requirement - Consent Letter (duly signed by all partners)
  7. 2 sets of completed Agency Agreement

All guarantors are required to submit the following documentation for agency application.

  1. Photo copy of Identity Card
  2. Salary Slip / Income Tax J Form / Bank Statement
  3. Particular of Guarantor(s) form duly completed
  4. 2 sets of completed Guarantee & Indemnity

Applications for people in the private limited category requires the following documentation:

Agency Application
  1. All directors will act as guarantors
  2. Photo copy of Identity Card of all Directors
  3. Memorandum and Article (Form 9)
  4. Form 24 - Allotment of Shares
  5. Form 49 - Particulars of Directors / Manager / Secretaries
  6. Company's Resolution on Agreement
  7. Form 32A-Form of Transfer / Debentures (if applicable)
  8. 2 sets of completed Agency Agreement

All guarantors are required to submit the following documentation for agency application.

  1. Photo copy of Identity Card
  2. Salary Slip / Income Tax J Form / Bank Statement
  3. Particular of Guarantor(s) form duly completed
  4. 2 sets of completed Guarantee & Indemnity
